Vajradhara with consort. This image coluld also be Vajrasattva Heruka with consort or Orgayn Dorje Chang - one of the eight forms of Padmasambhava.
Vajrasattva almost never has a thrid eye. Vajradhara can sometimes be found with a third eye and Orgyan Dorje Chang often has a third eye.
